Initial Contact & Inquiry

Schedule a Tour

Visit our home to see our facility, meet our staff, and experience the warm environment we've created. Tours can be scheduled at your convenience, and family members are encouraged to join.

Needs Assessment

We'll conduct a comprehensive assessment of your loved one's care needs, health status, mobility, dietary requirements, and personal preferences. This helps us develop an individualized care plan.

Financial Discussion

We'll review our pricing structure, payment options, and answer any financial questions. We believe in transparency and want families to clearly understand all costs.

Documentation & Paperwork

Complete admission paperwork including health history, medication lists, emergency contacts, care preferences, and financial agreements.

Move-In Day

We'll help coordinate a smooth move-in, including setting up the room and introducing your loved one to staff and other residents. Family members are welcome to be present.

Who is Eligible?

We welcome residents who:

Are age 18 or older
Require assistance with activities of daily living (ADLs)
Need non-medical personal care and support
Can benefit from 24/7 supervision
Are mobile or require mobility assistance (wheelchairs, walkers welcome)
Do not require skilled nursing or constant medical supervision
Can participate appropriately in a group living environment

We Accept Residents With:

Physical disabilities requiring assistance with ADLs
Mild to moderate dementia or Alzheimer's disease
Chronic conditions managed with medications
Recovery needs following hospitalization
Mobility limitations

Care Needs We Cannot Accommodate:

24/7 skilled nursing care requirements
Ventilator dependence or complex medical equipment
Severe behavioral issues that pose safety risks
Aggressive or violent behavior
Severe mental health conditions requiring psychiatric care
IV therapy or other skilled nursing procedures

To complete the admission process, please prepare the following:

Medical Information:
- Recent physical examination (within 30 days)
- TB test results
- Current medication list
- Medical history and diagnoses
- Physician contact information
Personal Information:
- Identification (driver's license or state ID)
- Insurance information (if applicable)
- Emergency contact information
- Power of Attorney or guardianship documents (if applicable)
Financial Information:
- Payment information
- Financial responsibility documentation

What to Bring on Move-In Day

Essential Items:

Clothing for all seasons (labeled with resident's name)
Personal hygiene items
Medications in original containers
Comfortable shoes with non-slip soles
Glasses, hearing aids, dentures (if used)

Optional Personal Items:

Family photos and personal decorations
Favorite blanket or pillow
Books, puzzles, or hobbies
Small personal furniture items (with prior approval)

Items NOT Permitted:

Weapons of any kind
Alcohol or illegal substances
Space heaters or other fire hazards
Large furniture (without prior approval)
Candles or incense

We understand that adjusting to a new home takes time. We encourage a trial period to ensure both the resident and family are comfortable with the care and environment. During this time, we work closely with families to address any concerns and make adjustments to the care plan as needed.
